Tuesday, April 11th: 8 a.m. - 10 a.m.
Friday, April 14th: 12 p.m. - 2 p.m.

Items to Bring with You

• Deed or lease
• 2 additional proof of residency
• IEP (if applicable)
• Custody documents (if applicable)
• Birth Certificate
• Immunizations
• Physical
• Negative TB Test
